An Independent Investigation commissioned by NHS England / Improvement (NHSE/I) following a Serious Incident in 2017 has been published. This involved a service user of North West Boroughs Healthcare NHS Foundation Trust (NWB). NHS Knowsley Clinical Commissioning Group and St Helens Clinical Commissioning Group have been fully involved with the investigation.
We welcome the findings in this report and we will work with partners to ensure that the learning will be used to improve the quality of care provided to patients.
