Independent Investigation Report Published

10th June 2016: Independent investigation into the care and treatment of P, a mental health service user.

Niche Patient Safety was commissioned by NHS England in September 2015 to undertake an independent investigation into the care and treatment of mental health service user P who pleaded guilty to the homicide of M, a 17 year old young man who was part of P's extended family.

A number of NHS Trusts were involved in caring for P at various stages from June 2013, up to January 2015, as follows:

• 5 Boroughs Partnership NHS Foundation Trust – provided community mental health services between June 2013 and October 2014 when P was imprisoned.

Mersey Care NHS Foundation Trust - provided mental health care to P while he was in HMP Liverpool between October 2014 and December 2014.

• Lancashire Care NHS Foundation Trust - whilst the Trust did not provide care to P during the span of this investigation, the recommendations concerning prison health care are directed to it as the current provider with effect from 1 June 2015.

Each organisation has produced an action plan in response to the investigation.
