Independent Investigation into the Care and Treatment of a Mental Health Service User

An independent investigation into the care and treatment of a mental health service user, ‘David’, in Wigan.

NHS England North commissioned Niche Patient Safety (Niche) to carry out an independent investigation into the care and treatment of a mental health service user ‘David’ Niche is a consultancy company specialising in patient safety investigations and reviews.

The main purpose of this independent investigation is to ensure that mental health care related to the death of ‘David’ are investigated in such a way that lessons can be learned effectively to prevent recurrence. The investigation process may also identify areas where improvements to services might be required which could help prevent similar incidents occurring.

The Executive Summary of the Report into the care and treatment of ‘David’ has been published today.

NHS Knowsley Clinical Commissioning Group (CCG) sends condolences to everyone affected by this tragic event and has taken steps to ensure that the learning from this event is shared across commissioned services to help prevent a similar incident occurring again.
